MERS Health Care Savings Program (HCSP)*

The City Hall Bargaining Unit has voted on and approved the following 3 options to submit to the City for a Letter of Agreement. In order to finalize the Letter of Agreement, we need everyone's decision on which option they which to select. Please make your selection and fill out the form by Thursday, August 27, 2020.

I elect to opt-in to the Employee 2% contribution with 100% leave conversion – any employee covered by this agreement cannot take a cash payout but can decide how much of their payout to contribute to HCSP and/or 457
I elect to opt-in to the Employee 1.5% contribution with 100% leave conversion – any employee covered by this agreement cannot take a cash payout but can decide how much of their payout to contribute to HCSP and/or 457
I elect to opt-out of the MERS Health Care Savings Program. (Employees who elect to opt-out of the MERS Health Care Savings Program (HCSP) understand they are not in a covered group as outlined in a HCSP agreement and will not be subject to the provisions (mandatory salary reduction and/or leave conversion). "If a Participating Employer desires to amend any of its previous elections contained in this Participation Agreement, including attachments, the Governing Body by official action must adopt a new Participation Agreement and forward it to the Board for approval. The amendment of the new Participation Agreement is not effective until approved by the Board and other procedures required by the Trust Agreement and Plan Document have been implemented.)

I agree with the above selection and understand that I cannot change my decision after I sign and submit this form.
